Urine Collection Instructions for UTI and STI Testing
URINE SPECIMEN PREPARATION

Complete requisition form. Make copies (both sides) of patient license & insurance card.

Print out patient label or fill out patient label provided in kit. Name on label should match requisition form. Label urine cup with patient name using permanent marker.

Wash hands, then dry. Put gloves on.
PATIENT LABEL
URINE COLLECTION (CLEAN CATCH)
Step 1: Remove lid from urine cup. Clean genital area with towelette.
*Females: Separate the skin folds around the urinary tract opening and wipe from front to back.
Urinate a small amount in toilet. Collect 30 mL of midstream sample. Place lid on cup and secure.
Step 2: Place cup on flat surface. Peel back label on lid for transfer device. Insert empty tube into cavity of lid, stopper down. Advance tube over puncture point to pierce stopper.
Step 3: Hold tube in position until flow stops (7-9 mL). Gently invert the tube 8-10 times to mix the sample with the additive. Place patient label on tube.
SHIPPING INSTRUCTIONS

Place specimen tube inside biohazard bag & seal bag.
02
Place requisition form & patient forms in pocket on outside of biohazard bag.
03
Place biohazard bag inside prepaid return mailer & seal.
04
Schedule pick up from shipping provider or drop off at a local dropbox. Ship same day for faster results.
