Collection Instructions for Nutrigenomics Testing
PREPARATION FOR PATIENTS
DO NOT eat, drink, smoke, or chew gum 30 minutes prior to collection
BUCCAL SPECIMEN PREPARATION

Complete requisition form.

Fill out patient label provided in kit. Name on label should match requisition form.

Wash hands, then dry.

SPECIMEN COLLECTION
Step 1: Peel open packet and set aside Dry Pouch (step 2) and barcode sticker (step 3). Remove buccal swab without touching the swab head and hold in hand.
Step 2: Firmly rub swab head against inside of both cheeks for total of one (1) minute. Place swab in Dry Pouch provided with swab head down.
Step 3: Close by folding at “fold line” and seal with patient label. Place barcode sticker on outside of Dry Pouch beside the patient label.
SHIPPING INSTRUCTIONS

Place the specimen Dry Pouch inside the biohazard bag and seal bag.

Place requisition form in outside pocket of biohazard bag.

Place biohazard bag inside prepaid return mailer and seal.
04
Schedule pick up from shipping provider or drop off at a local dropbox. Ship same day for faster results.
